We get calls every day from people looking to get a copy of their birth certificate. Depending on your timeline, here are some details on how you can obtain the record you need. The Arizona Department of Health Services Bureau of Vital Records no longer provides same day walk-in customer service for certificate issuance. You may apply for a certified copy of a birth or death certificate by mail. If you would like same day walk-in customer service please visit your local county health department Office of Vital Records.
However, there are specialty services that the local county health department does not perform. Those services can be obtained by scheduling an appointment with our Bureau of Vital Records.
Specialty services include:
- Delayed Birth Registration
- Foreign Born Registration
- Adoptions
- Putative Father Registrations and Searches
- Corrections or Amendments of records for births that occurred prior to 1997 and deaths that occurred prior to 2008

If you are looking for a birth or death record from another state, you must contact that state’s vital records office. Birth and death records for events that occurred in other states are not available from Arizona’s Bureau of Vital Records.
