It’s time for back to school and enrollment for fall sports, both of which will likely require you to submit a copy of your child’s birth certificate. Our Bureau of Vital Records is responsible for maintaining and issuing certified copies of Arizona vital records, including birth certificates.
Directions on how to apply for an Arizona birth certificate can be found on our website. Please note, each state maintains its own vital records. If you are looking for a birth or death record from another state, you must contact that state’s vital records office. Birth and death records for events that occurred in other states are not available from Arizona’s Bureau of Vital Records.
