I am concerned that without the notarization process of application paperwork, et al, and the option of sending certified mail, that many very ill patients can be made even more vulnerable, due to lack of technological knowledge. Since this entire endeavor is new and fluid, I am hopeful that AZ DHS staff will recognize the necessity of options being provided to citizenry who are sick and/or disabled. Even the Social Security Administration allows a variety of application methods/venues. I am an advocate for those human beings for whom most other medications have ceased to be effective. Please rethink the online-only application process; it is not always the most secure nor expedient method.
Particularly when the online application process does not work…
I misspelled my Doctors name by one letter, it’s correct on the ADHS recommendation though, is that an issue?
I know another person who put in the wrong address for their caregiver, how do they remedy that?

If there is a problem with your form, it will be sent back for you to fix it. If not, it will go through the way you submitted it. Later there will be an application on the website to make changes for a $10 (ten dollar) fee.
Does anyone have any information as to the cost of a prescription? If I can’t afford the prescription cost then all this preliminary certification is superfluous.

The cost will be up to the dispensaries, this is not our jurisdiction.
From what I have found, prices in California seem to have stabilized at between $300-400 per ounce. A lot will depend on the dispensary, strain, availability, etc., and may be higher initially until everything settles down. For a quality strain at the legal allowance, you could probably expect something in the region of $800-$1000 per two-week period if AZ prices settle at the same averages as CA. For people who cannot afford these prices, the whole MMJ legalisation issue is going to be a moot point unless the laws are changed to allow you to have the option to grow your own or purchase from a dispensary as needed (as is the case with beer and tobacco–brew or grow your own, or purchase from the relevant store).
Until dispensaries open you can apply to cultivate your own MMJ.
If you living more than 25 miles from a dispensary, you can have a registered caregiver help you with your MMJ supply.
I am sure Mr Humble will correct any errors with the last two points.

No, it is not a requirement but may be relevant if there are more than one applications in a given CHAA. All principal officers, board members and dispensary applicants must have lived in Arizona for the preceding three years before application. According to the rules, an applicant who is delinquent on federal, state, or local taxes, child support, or student loans, has an unpaid judgment due to a governmental agency, or a past bankruptcy can still receive a dispensary registration certificate. However, ADHS may use these factors as criteria in the selection of who will be allocated a dispensary registration certificate if there is more than one qualified applicant for a given CHAA.
If you do not have the $150,000, you may wish to apply for a more rural, less competitive CHAA.

I had completed my application on April 14 and was approved as a patient the same day. When I received my temporary card today there was an error on the card and I can’t easily correct it on your website. How does someone go about submitting any change requests to the Medical Marijuana ID card?
Your department made this error and now I have to correct it and of course your website hub is silent on how to submit this change within 14 working days. I should get a refund of my $150 because you totally screwed up my card by incorrectly stating I’m not approved to Cultivate? I checked the correct box to cultivate and you messed this up. If you don’t have this corrected from the 10 days I submitted my application I want my $100 refund because it is your error.

How do I get a picture of me to you on line for the MM card? I know how to upload, but how do i get the sizes you want?
You can use photo editing tools available in most photo viewers to size the pictures.
The Patient Attestation Form available on your site has a small issue – Arizona is spelled wrong…

Is there any hope for this process? Today is April 19. Last blog post was April 14, attributing the difficulties to traffic on the site. Really, do any of us stand a chance of getting our application submitted? I tried all day yesterday, last night at midnight, and again today.

In response to JD’s question, I left some information on the possible prices for the prescription based on current California prices, now that everything has settled in that state.
Is there a reason that post is still awaiting moderation even though a number of posts made since are now showing up?
Was there something wrong with giving information on what is known of current prices?

Mr Humble, I am with the Arizona Cannabis Society. Over the last few days we have been assisting patients with the online application process.
After many successful submissions, one application came back to the patient in the form of an email with an error on one of the forms. We have assisted in fixing the problem, however when attempting to resubmit, it is not accepting the Application ID # that is stated in the Email. Is there a technical department that may be able to assist us?

I am hearing so many conflicting opinions regarding Scottsdale use permit !!!! My Attorneys said that if we don’t get a letter from Scottsdale saying we met all qualifications…. that when we apply in June they will deny us until we do ….. is thi correct for Scottsdale? Should we move forward and spend the $ 20,000 before we know if we have a permit? Thank you for your time and clearing up this confusion !!!
ADHS rules require documentation from the city as well as an attestation from the application that the location meets local zoning.

I filled out and uploaded all necessary info but got an email telling me some item needed to be corrected. I used the link AZHD gave me. The problem is that nothing is highlighted to correct, the whole form is empty. This really needs to be corrected because it is redundant to have to fill in all this info AGAIN!! I pretty sure everything I entered the SECOND time it the same as the FIRST!!!!!

I wanted to thank you for the hard work and quick turn around time your department has shown in the past week regarding my application. Your online support has been very helpful, and I was able to resubmit my application without issue the second time around. As an IT Professional, I understand its inherent that any new system that involves a detailed work flow will always have a few kinks to work out. I, like many others, do not have the proper cultivation status but I understand this an error within the site rather than from the people receiving and handling the applications.
Do you know, if after we correct our cultivation status with your online support, that the proper status will be reflected in your database if law enforcement were to look it up?
If not, does this mean we are to wait until we receive our Permanent Cards with the proper status?

Once you are approved, the system will be updated and a card will be issued.
My first attempt to register did not go through do to my dr. making an error, an when I re submitted my corrected paper work there was not a box to check for cultivation as there was on my prior attempt to register. today when I revived my temporary card it stated “not approved to cultivate” how do we proceed to correct this error?

Will…. Thanks to you and your stall at AZDHS for all of the hard work involved in a short amount of time. I applied for my AZMM card and my application was approved very quickly, within 30 minutes so it appears the system is working.
While it would have been a benefit to have MM medication available for those who are registered, unfortunately no dispensaries have opened yet, and the only option is to cultivate. This is an acceptable option, but can you tell us where we can buy starter seeds or clones legally? I know you mentioned that maybe patients can get it from a magazine classifieds, but the only ads that I’ve come across are from seed suppliers in the Netherlands. This is an important question as we all want to be compliant with the law.
